Once we’ve carried out your Needs Assessment, we’ll create your bespoke Care Plan together, tailored to your needs, abilities and wishes. It will set out how your care and support needs will be met including any financial support you’re entitled to. This is your chance to have your say about the type of care and support you’d like. Everyone in receipt of care should have a Care Plan regardless of how your care is provided.
